A NEW Process is in place so please use the following guidelines as you register.

Follow these EASY steps:

  1. Click (or tap) the JotForm membership button.
  2. Choose Individual or Institutional Membership and complete requested information. (Institutional Membership requires a spreadsheet that is accessible from the JotForm).
  3. Invoice and Copy of Registration for comes directly to the inbox of the contact person from JotForm.
  4. Pay according to your preference as indicated on the Registration Form and Invoice.
